1) AccountForms.pdf = all forms necessary to open any type of account, pages that need to be completed:

• Pages 3 and 4 account holder information
• Page 13 Signature
• Page 14 Social Security number and signature
• Page 16 Signature
• Page 31 Signature
• Page 37 Income and net worth verification signatures

2) TradingAuthorization.pdf = Limited Power of Attorney for Managed Accounts.

• Page 1, Forex Capital at the top, signature on the bottom
• Page 2 acknowledgement

3) FCMAdvisoryAgreement.pdf = Forex Capital Agreement pages

• Page 7 is redundant and not necessary with the PFG accounts
• Page 8 customer name on top, signature at the bottom
• Page 9 managed account program, funding and risk parameters next, for example 100K initial ( amount invested ) 70K maintenance, ( maintenance = the equity stop on the account, the point at which all positions would be liquidated ) , customer signature at the bottom
• Page 10 acknowledgement only, introducing broker information and dealing center is left blank ( unless introduced by a 3rd party to FCM )
• Page 11 Authorization to pay quarterly incentive, signature at the bottom.
• Page 12 limited trading authorization, signature at the bottom
